Start with the building’s schedule
We offer SS11 fume cupboard testing for Form 12A purposes. The relevant starting point is the compliance schedule for your building: it identifies the system, performance standard and inspection, maintenance and reporting procedures. Ask the building owner or facilities manager for the current version before arranging the visit.
MBIE explains that a Form 12A can be issued only when the relevant schedule procedures have been fully complied with over the preceding 12 months. The owner’s annual BWoF is a separate declaration supported by the required IQP certificates. Confirm the IQP’s registration for the system with the relevant council.
Build a useful testing pack

Arrange the visit around laboratory work
Nominate one site contact who can obtain records and organise access. Tell FumeCare how many cupboards are involved, which rooms they serve and whether roof or restricted-area access needs arranging. Describe any active experiments or operational constraints so the visit can be scoped before attendance.
Record missing documents openly. For example, if a cupboard was repaired mid-year, send the repair report and identify any subsequent testing records you hold. Ask us what further records are needed to complete the testing pack.

- Who arranges access, shutdown coordination and any specialist attendance?
- Are there outstanding faults or recommendations to investigate before certification can be considered?
- Who will receive the inspection report, any remedial scope and the Form 12A if it can be issued?
Inspection frequency is determined by the applicable schedule, not by this checklist. Council examples may include owner checks and more frequent IQP tasks as well as annual reporting. General preparation guidance should be read alongside your own schedule.
